The lines that run from the side of your nose to the corners of your mouth, often called smile lines or nasolabial folds, are one of the most common things people ask us about. In this video you can watch a whole nasal fold filler treatment, start to finish, filmed on two cameras at our Brentwood clinic. Sam treats Mandy from our team, while Paul, our Clinical Director, talks through every step.

The aim: soften the line, not change the face
Good filler work in this area should be subtle. As Paul puts it in the video, we don't want to change the structure of the face, we just want to soften that line. Mandy's folds were not deep, so only a small amount was needed.
The treatment, step by step
- Planning. The fold is checked sitting up, so you can see exactly where the line sits.
- The first pass. The needle goes in at about 45 degrees at the top of the line, and the filler is placed slowly as the needle is drawn back. Starting high also gives the cheek a subtle lift.
- Comfort first. Three tiny amounts are placed first so the area is numb, which makes the main pass far more comfortable. Mandy was so relaxed she nearly fell asleep.
- Moulding. With a gloved finger inside the mouth, the filler is smoothed into the right place. Sam asks before she does it, as you always should.
- The second side. A fresh syringe and needle are used for each side.
- Finishing. The line should feel smooth under the finger, with no lumps or bumps.
What to expect afterwards
- A small spot of bleeding at an entry point is normal, and gentle pressure stops it.
- The result is visible straight away and looks softer, not filled.
- If a firm spot appears, it can usually be eased with gentle warmth. Your practitioner will advise you.
- Results vary from person to person.
Is it right for you?
Dermal filler is for adults aged 18 and over. Everyone has a consultation first, so we can check your skin, your medical history and what you would like to achieve, and give you an honest view of what is realistic. If filler isn't the right treatment for you, we will say so.
