Filler Tweaks in Brentwood: Why Small Top-Ups Beat Waiting for It to Wear Off

A lot of people think of dermal filler as a one-off: you have it done, it lasts a while, it wears off, and then you start again. On a recent TikTok Live from our clinic on Kings Road, Brentwood, Paul, our Clinical Director, sat in the chair so our nurse Bina could show a different approach. Instead of waiting until all the filler has gone, you come in for small tweaks along the way.

Miss B is our AI treatment guide. The treatment footage is real, filmed live with consent. Results vary.

Filler does break down, and that's normal

Hyaluronic acid filler is designed to be broken down by the body gradually. How quickly depends on the area, the product and the person. If you wait until it has completely gone, you are back to square one and often need a larger amount to rebuild what you had. Small top-ups placed before that point can help maintain the look with much less product each time.

Tiny amounts, placed where they're needed

In the video Bina treats several areas, each with very small amounts:

  • Lips: a little hydration for the colder months, to soften a dry, crepey texture rather than change the shape.
  • Oral commissures: the corners of the mouth can pull downwards over time, which creates a heavier, sadder look. A small amount here can soften it.
  • Nasal folds: Bina uses a fanning technique to soften the folds without adding obvious volume.
  • Cheeks: placing product higher up gives a lifted, contoured look rather than puffiness.

In total, Paul's tweaks used about 1.5 ml across all of these areas.

Why a finer needle matters

Bina decants the filler into a finer needle than the one supplied in the box. A finer needle gives more control over how much goes in, which helps avoid overfilling. It can also mean less swelling and bruising. It is one of the techniques we teach in our advanced masterclasses for clinics.

Little and often keeps you looking like you

The aim of a tweak is not a dramatic change. It is to keep you looking fresh and like yourself, rather than going through a cycle of "full" and "worn off". Many patients find that small, regular reviews feel more natural and are easier to fit around life.

Is a tweak right for you?

Everyone's face, filler history and goals are different, so we always start with a consultation. We look at what filler you have had, where it is now and whether a tweak, a top-up or nothing at all is the right call. Sometimes the honest answer is to wait.

Frequently asked questions

How often should I have a filler tweak?

It depends on the area, the product and how your body breaks it down. We will suggest a review interval at your consultation rather than a fixed schedule.

Will a tweak make me look overdone?

The point of a tweak is the opposite. Tiny amounts are used to maintain a natural result, not to add volume.

Can I have a tweak if my filler was done elsewhere?

Yes, but we will assess it first. We need to know what was used, where, and whether it has moved before we add anything.

Are you a clinic wanting to learn this?

We teach these techniques in our advanced filler masterclasses. Trade accounts and products are available to approved clinics at 65medical.co.uk.
